Adoption Application

Please be aware that because AZBHR is an all volunteer organization, it may take up to two weeks, from the time your application is received, to coordinate a home visit and complete the adoption process. - And finding the right new family member is well worth the time it takes!

NOTE:  If you use the Word or Adobe versions, you must save these files to your local hard drive (choose Save when prompted) then fill them out and send them to us via "Snail-mail" (you know - The old fashioned way, with an envelope and a stamp).
